1) Scoliosis screenings, for all students over the age of ten, will begin the week of May 5, 2008. Please return all permission slips by Friday May 1, 2008. 2) Immunizations requirements will be changing starting in September 2008. Children born on or after January 1, 1997and/or entering the 6th grade will need one dose of Tdap and one dose of a meningococcal vaccine in order to attend school. Children entering Pre-K will need one dose of Influenza vaccine and one dose of Pneumococcal conjugate vaccine. |

3) In accordance with New Jersey law, all pupils in the third grade must have a current physical exam completed by a physician. School forms will be mailed home to each third grade student. Physicals were due April 28, 2008. Please send if all forms ASAP. 4) All 6th grade students entering Leonia Middle School will need a complete physical examination prior to starting in September. Physicals are due in the beginning of June. |

5) Students are not allowed to have medications, of any kind, in school. If it is necessary for your child to receive ANY kind of medication during school hours please contact me so that the proper forms can be completed. |

6) Just a reminder! If your child will be out of school please call the Nurse's office, before 8:30 AM to report their absence Also, according to school policy, a student must be fever free for 24 hrs before returning to school. |

7) All students will be screened for height, weight, BP, vision, and hearing. Students 10 years of age and older will be screened for scoliosis. If for any reason you want your child to be exempt from these services, please notify the health office in writing. |
| 8) NJ State Law now requires that a person under 17 years of age wear a helmet when bicycling, roller skating, or skateboarding!! |
